POSITION TITLE: WOUND CARE NURSE/STAFFING COORDINATOR
POSITION SUMMARY: The Wound Care Nurse assesses, treat, and create care plans for patients with complex wounds, ostomies, or continence conditions. Implements treatment plans. This position relies on the Director of Nursing (DON) for guidance.
RESPONSIBILITIES:
STAFFING COORDINATOR -
Prepares weekly/monthly staff schedule.
Resolves missed punches for Nursing in the HRIS system on a daily basis.
Reports changes/terminations and updates open position board for recruiting purposes.
Follows-up with New Hires to ensure they are informed of start date and training/orientation dates.
Manages “Train-the-Trainer" program
Ensures Training/Skills checklist(s) are completed for all Clinical New Hires.
Tracks TB/CPR/Flu shots to ensure staff is immunized annually.
Ensures Nurse and CNA licensures are up-to-date (not expired) to ensure compliance.
Conducts New Hire Orientation (Onboarding/New Hire Orientation for All staff.
Communicates as necessary with Nurse Management and rotates On-Call phone with Nurse Management to ensure adequate stand-by staffing.
WOUND CARE-
· Cleans wounds to eliminate bacteria and debris and reduce infection risk.
· Chooses the best treatment for a wound like regular cleaning, antibiotics, etc.
· Bandages the wounds by creating a barrier against microorganisms and changing the dressings regularly.
· Educates patients and caregivers on how to manage and dress wounds to aid in healing and to accelerate wound healing and prevent skin disintegration.
· Provides infection and injury prevention and pressure ulcer (bed sore) care education to patients and caregivers.
· Collaborates with other patient care team members to decide the need for any additional therapies or adjuvant care.
· Completes accurate documentation on patient/resident condition including assessments.
· Effectively communicates with physicians, Nurse Management, and other members of the care team.
· Attends Care Plan Meetings.
· Ensures safe work practices are followed.
QUALIFICATIONS:
Must be a licensed nurse (RN or LVN) with active, applicable state licensure.
Prior experience in healthcare/LTC preferred.
· Nursing experience in a long-term care facility is preferred.
· Supervisory experience is HIGHLY desirable.
· Effective communication and interpersonal skills are necessary.
· Ability to speak, read, write and understand English.
· A valid TB test and BLS/CPR card.
· Ability to lift at least 51 pounds on a regular basis and physically able to help residents who require physical assistance.
OTHER QUALIFICATIONS:
· Must possess a general appreciation of seniors and an understanding of the aging process, as well as be aware of its challenges.
